Truemed works directly with licensed healthcare providers to determine eligibility and, when appropriate, issue the required documentation—such as a Letter of Medical Necessity—based on individual health considerations.
This partnership allows the eligibility process to remain structured, compliant, and independent from the training itself.

Book your service — Choose your membership, package or session plan with Elevate Fitness as usual.
Complete the Truemed survey — Use the link below to take a short clinical intake form via Truemed. A licensed practitioner will review and issue a Letter of Medical Necessity (LMN) if you qualify.
Submit for reimbursement — With your LMN, request reimbursement from your HSA/FSA administrator. If approved, you’ll effectively pay with pre-tax funds, increasing your savings.
Note: Elevate Fitness does not accept HSA/FSA funds directly online; clients pay normally and then pursue reimbursement via their HSA/FSA plan.
